2015/16 Dependent Checklist

All forms that are not submitted on the web or electronically signed, must be submitted to the NAU Financial Services Office through one of these methods:

  1. All Students-Federal Student Aid Username/PIN - www.fafsa.gov
    If you or your parent do not have a Federal Student Aid PIN, you each will need to apply for one through the FAFSA website.  You will immediately receive a conditional PIN on the website.  Be sure to keep and safeguard your PIN, you will need it for future years and other processes.  You and your parent will each use the PIN to electronically sign your Free Application for Federal Student Aid (FAFSA) and any Loan Master Promissory Notes.

    1. On May 10, 2015, the FSA Username and password will replace the PIN for students and parents.  If you are a new user May 10, 2015 or later, you will automatically be directed to setup your Username and password.  If you already have a PIN, and access the FAFSA, StudentLoans.gov, or NSLDS website on or after May 10, 2015, you will be directed to setup your Username and password and the information will be matched to your previous PIN information.
  2.  All Students-Free Application for Federal Student Aid (FAFSA) - www.fafsa.gov
    1. If you have already completed your FAFSA for this award year, please use your PIN and make a correction by adding our school code, 004057 Rapid City SD (for all campuses), on the FAFSA website.
    2. Every award year your will need to apply for Federal Financial Aid, please use the 2015/16 year.  Use the IRS Data Retrieval Tool to download your 2014 Federal Tax Return information directly to your FAFSA!
      1. If your taxes were filed electronically you can use the IRS Data Retrieval Tool 2-3 weeks after you filed.
      2. If your taxes were filed by paper you can use the IRS Data Retrieval Tool 8-10 weeks after you filed.
  3. Submit the following forms:
    1. New Students and Students who have been away from NAU one or more quarters or withdrew during the most previous quarter-Submit form through NAU Website
      1. NAU Supplemental Financial Information
    2. All Students-Either electronically sign and submit, or print, complete, sign and submit by fax, scan or physical mail
      1. PLUS Information Request
      2. If Student or Parent did NOT file Taxes
  4. All Students-You will receive notification to retrieve your Student Aid Report (SAR) by e-mail within 48 hours, if you provided your e-mail address on your FAFSA.  Review all of your answers and notify us if you have any corrections to be made.
  5. Undergraduate Minnesota Resident Students ONLY-If you are attending the following campuses:  Bloomington, Brooklyn Center, Minnetonka, Rochester, Roseville; or if you are an OnLine student living in Minnesota.
    1. MN Direct Deposit Authorization-Electronically sign and submit
    2. MN State Grant Application-Electronically sign and submit
    3. MN Child Care Grant Instructions
    4. MN Child Care Grant Application-Print, complete, sign and submit by fax, scan or physical mail
  6. STOP HERE:  Do Step 6 after you have received your Student Aid Report (see step 4).
    1. Students:  Previous Direct Loan Borrowers regardless of which school attended DO NOT need to complete Step 6.
      Parents:  If you have previously borrowed a Direct PLUS Loan for THIS student, you only need to “Request a PLUS Loan”.

      2. Regardless of which campus you are affiliated with, be sure to choose the “School State” as South Dakota and the “School Name” as National American University (no city listed).  This should bring up NAU with a Rapid City, SD address.  If you do not choose the state and school correctly, you may have problems continuing with the Master Promissory Note (MPN) process.
      3. Student and Parent:  Select “Sign In”.  Enter your information and sign in with your Federal Student Aid PIN/Username.  Verify your personal information
      4. Student:  New Student Loan Borrowers:
        1. Choose “Complete Entrance Counseling”
        2. Once entrance counseling is complete, chose “Complete Master Promissory Note” and then click on “Subsidized/Unsubsidized”
      5. Parent:  New PLUS Loan Borrowers:
        1. Chose “Complete Master Promissory Note” and then click on “Parent PLUS”
        2. Once you have completed the PLUS MPN, you will need to request a PLUS credit check by choosing “Request a PLUS Loan”
      6. Parent:  Previous Direct PLUS Loan Borrowers:
        1. Chose “Request a PLUS Loan” to request an updated PLUS Loan credit check
