| All students taking an online course through NAU Distance Learning are responsible for selecting a qualified proctor (whether campus or individual). NAU is not responsible for anyone who does not select a proctor and consequently misses any proctored exams administered in their courses. |
Make sure that you use the TAB key to advance. If you click on the enter key you will actually submit your form before you are ready and will have to start over.

Thank you for carefully reading all of the following important information before completing the Proctor Request Form found below and submitting it.
As a distance learning student, you will take some of your exams by yourself working on the computer; and you will take some of them in the presence of a qualified person who gives you a hardcopy exam or uses a password to open an online exam on an internet accessible computer. You are responsible for finding this qualified person to become your proctor. You will take all of your distance learning proctored exams in the presence of your proctor who will administer them.
PLEASE NOTE:
- Students registered with one of the NAU onground campuses must use that campus as their proctor.
- Students registered with the distance learning (online) campus must use any onground campus they are near as their proctor.
- Students registered with the distance learning (online) campus living near Rapid City, SD must use the distance learning campus as their proctor.
- If you qualify for any of the above three categories, please fill out the Proctor Request Form below, and put the name of the campus, as listed at the top of this form, into the "Proctor's Full Name" box on the form.
- If situations 1-3 above DO NOT INCLUDE YOU, you will need to select an individual, qualified proctor who will test you wherever you are living and working. If you are selecting an individual proctor, you are responsible for ensuring that the proctor you choose meets all requirements.
- Possess a bachelor's degree.
- Be a neutral person (NOT a past, present, or future family member OR a friend).
- Have access to a non-public fax machine. Fax machines open to the public such as Office Depot/Max or Kinko's are not to be used.
- Have a valid e-mail address.
- Have a computer with internet access available for a maximum of three hours per online proctored exam
- High school, college or university personnel.
- Corporate training personnel.
- Public library personnel.
- Immediate supervisor.
- Pastors and assistant pastors
- Military education officer or military personnel with a higher rank than the student.
- Testing center personnel
- Act as an unbiased proctor.
- Verify the identification of the student BEFORE giving an exam
- Ensure the completed exam is solely the work of the individual student
- Continuously monitor the student during the exam
- Maintain exam confidentiality.
- Immediately return completed cover letter and exam answer sheets to NAU; for each hardcopy exam the student takes - (1) FAX each exam's cover letter and answer sheets as a SEPERATE FAX; and (2) MAIL all the hardcopies back in that exam's return envelope.
- Determine a date, time, and place to complete your proctored exam by setting up an appointment ahead of time. If your proctor is an NAU campus please locate the phone number to each campus found on the first page of this form and call the campus at which you are testing.
- Be sure your Proctor received the appropriate hardcopy exam for written exams or online proctored exam instruction sheet for proctored exams taken on the computer.
In order for timely delivery of your exams to your proctor (campus or individual), you must ensure that the following contact information you are providing for yourself and your proctor is complete and accurate.
